Tell him you've come dangerously close to swatting him with your newspaper a couple of times because you thought he was a fly with all his humming noises. Tell him it's for his own safety not to do that anymore.

Just ask him what song he is humming. Your coworker probably does not even realize that he is humming, that you can hear him or that anyone might possibly be bothered. You do not have to confront him on the issue and make something out of nothing; just ask, every time you notice, "What song is that?"
He will quickly come to understand that he does hum almost continuously, and that other people can hear him. He will stop out of sheer self-consciousness and embarrassment, without you seeming like a jerk.

Ask him what the tune is and if it's nothing, just him let him know you need to concentrate and he keeps getting that darn song in your head! Otherwise bring an iPod to work and block it out (or earplugs). If this collegue sees this he may get the hint.

Tell the coworker that you have been having difficulty concentrating lately. Ask him if he would refrain from humming, because it is affecting your work. If he does not, speak with a supervisor. Ask that you be moved to a quieter area. If the supervisor asks why, tell him/her that the humming is affecting your ability to work.

Can you bring in a little radio and turn it on? Person starts to hum, flick the radio on, eventually he will get the hint.
