Your best bet is to file for an extension, that way you'll have the time to put together all the deductions you have, and you won't have a CPA who is rushing to get your taxes done. The only problem with filing for an extension is that you're supposed to have all your tax liability paid or overpaid by the filing deadline.

It really depends on the area you live in, since the charges are vastly different. If you have a lot of deductions and a business, there is much more work involved. Have you tried the Turbo Tax software? If you don't have a lot of investements there is one for that. If you have quite a number of investments there is a Turbo Tax program for that also. It walks you through for all scenarios and is quite comprehensive although easy to do.

That depends upon the part of the country and how much time is involved in preparing your return. You can expect to pay a premium for a last minute job.
