No. Many new converts take a Confirmation name (the name of a saint) upon joining the Church, but that won't be part of your legal name, and you won't have to change your last name. I'm Catholic. Although I took a Confirmation name, I have an Italian last name, and didn't have to change it. Read more about the practice of Confirmation names here:

No. You can keep your surname; however, in baptism and in confirmation, you usually add a name that follows your first name.

Baptism is a separation from your previous life to your new life in Christ. An addition name is there as a reminder.

No, u don't have too. I have indian friends who are christians and muslims who keep their indian last names. its your etnicity label, u shud uphold the names. I'm a native and catholic since birth and i keep my ancestral names.

What gave you the idea that you would have to change your name? If a Christian became a Hindu, would they have to change their name?

one of my college professors and her sister have arabic names and her sister is tall and looks arabic, and about a week after 9/11 they had to take a flight from Boston to LA, the exact route traveled by the terrorist. You CANNOT imagine the scrutiny she talked of having to endure. It was incredible.

No. That would imply a legal procedure that has nothing to do with religion and it would be your choice, not a religious based one.

No but as a former Catholic and now a Hindu and knowing how spiritually richer and stronger I feel now, I can't help but question why on earth would you want to??
