Ronk Electrical Industries specializes in phase converters. Check out their web page.

The second link is a wiring diagram of a rotary phase converter.

forget it. buy a solid-state phase converter. figure about $100/hp with the price decreasing as HP increases up to 20 HP. I built a rotary phase converter and tuned it best I could by varying the capacitance. The phase voltage and current was wild in the best of configurations. I talked to a manufacturer about them and they said theirs was wild also, nothing could be done about that. The phase converters work great and they have built in features like overload protection, speed control, reverse, speed ramp, brake, volt meter, amp meter, etc.
Looks like you got about 15 HP. A Westinghouse TECO FM100-215-N1 will do the job. But listen. Some motors can’t be operated with a phase converter. A big caution note was in the box my new 3 HP motor came in. It said “ caution do not operate this motor with a phase converter”. It was correct. I was driving the motor to max but it lasted about 3 years anyway. I've been powering other motors some 50 years old without a problem. I stoped over driving motors after burning up my experimental motor. What is this beast driving anyway? How many HP do you need from it cause there is another way to do this but you will loose 25% HP. It.s called a HAS phase converter. I have one 30 yo and only replaced one start capacitor.

GFI's are for personal protection so its up to you. Obviously a ground fault on the secondary side of the converter will not be detected on a GFI connected on the single phase. Incidentally a GFI on the secondary will not work unless the Wye point is grounded anyway.
