It's hard to give a specific answer because different countries measure their recycling wastes differently. Canada and the US for example include composting as part of recycling whereas other countries don't. When it comes to plastics recycling the US publishes figures for plastic bottles whereas other countries include all types of plastics. Figures are also released periodically so whilst there's up to date info for some countries it has to be compared to info from other countries that may be several years out of date. Some countries publish figures based on the percentage recycles, others go by weight.

Generally Scandanavian and north western European countries recycle the most. Holland recycles the most domestic waste - some 39%. Overall the countries with the highest recycling rates include Sweden, Finland, Norway, Holland, Denmark, Austria, Switzerland, Germany, France and Japan.

There's a shortage of the type of information you're looking for but these pages (both pdf files) may help... http://www.environment.gov.au/soe/2006/publications/drs/pubs/576/set/hs57nolan-recycling-comparison-report.pdf and http://recyclingnearyou.com.au/documents/TheRecyclingOlympics-2004.pdf

Good question. I'd google 'Germany recycle' and go from there. They have been on top of this for a long time.

100% except for some of the atmosphere and electromagnetic energy zinging away. I doubt we can use depleted uranium again for anything but armor penetrating bullets but hey that is recycling too.
