Last name developed over time. Sam who lived on a hill became Sam Hill and his children took the last name Hill but Tom that lived on a hill also became known as Tom Hill and his children became know as hill. Man named Jack had a son and his sons became known as Jackson and their children took the name Jackson. But there was more than one Jack.
People with the same surname are not necessarily related. A person who displays a coat of arms just because it has their name is like someone putting up a picture of a famous person with their name and claiming him as their ancestor whether they know he is or not.
Some names came from occupations. Tom the Barber became Tom Barber and his children took the name Barber. Other occupational names were carpenter, clark(clerk), smith, jones etc.\
Some were named for the towns they came from or if they lived close to a castle they took the name of the castle (family that lived there) whether they were related or not.

The first group to utilize set or perminent surnames are the Irish. This happened about 1200. Then the name would be based on clan connection, occupation, personal habits/features, or geography. This spread to Scotland, England, France, Scandavia and so forth. For instance my surname of Donaldson (Son of Donald) is derived from my Clan-the MacDonald's of Scotland. The name's spelling has changed from the orginal Gaelic to this form. Orginally it was MacDomhnaill. This was used to distinguish clan members from non clan members. Different trade also passed on thier names to the children-Smith was orginally a black smith, geographic basis would be like the O'Dells of Ireland. Orginally this name was Woodelll-after moving into Ireland the name was altered to its present form. A woodell is a clearing in a wooded area. Hope that this answers the question. For the most part the names were set by the community a person lived in or by a government.

Our parents gave us our first and middle names. Sometimes our middle names are part of our parent's maiden names.
The last name is from our ancestors.
BUT...If you are a celebrity or a celebrity parent, the studio or our celebrity parents, gives us our stage names.
If we are not celebrities and change it, then our names would just be alias names.
In the olden days, like the guy or gal said above , people used first names and origin for identification. So I guess it came from the officials that named them.

Surnames are derived from geographical origin of people, partcularly the families they came from and the occupations which they had. In Ireland for exmple O'Riley means of the Riley clan. In Scotland MacDonald means relative of the Donalds or in Spain and France when 'de' is used before a name it means that you are froma certain place.

Early times were mostly agrarian societies, with people spread out over the land. Only first names were used to differentiate between people, even between clans. If they needed more specifics, they tacked on their place of birth, such as "Jesus of Nazareth" or "Leonardo da Vinci." Eventually, these first names were used over and over again, forming a pool of specific names parents could use - and only use.
