a valid passport with at least two blank pages. The Maltese passport must be valid for the duration of the intended stay in the U.S.

an ELECTRONIC VISA APPLICATON FORM DS-156 duly filled out and signed. You can access the FORM by clicking on the following link to the electronic visa application form and present the completed form at the moment of the interview. Please do not send this form to the American Embassy by email or fax.

one recent photo with a white background, taken within the last 6 months. Size : 5 cm by 5 cm. Please note that your visa application will not be accepted for processing if the photo dimensions are not correct. To check photo requirements, click here.

an original receipt of payment of 36.50 Liri (visa application fee) per applicant. This fee must be paid in advance at any Bank of Valletta branch. This fee must be paid before the visa application may be submitted and cannot be reimbursed, even if your visa is denied

a valid freedom of movement stamp from Malta in passport, if you are not a Maltese citizen.

Note: Some nationalities (but not Maltese) need to pay an additional visa issuance fee based on reciprocity between the United States and the country of nationality of the applicant. The visa issuance fee can be paid at the Consulate in cash either in dollars or Maltese Liri.

Step 2: Go to the Embassy for the interview. You present yourself at the consular section with the required documents (see list above).
