I wanted to change my middle name to my maiden name and take my husband's family name as my last. I'm in the process of renewing my passport and filled out the application as stated above. Did I need to go to court and get it legally changed?

Many states allow the name change to happen for free at the time of the marriage, based on what you fill out on the application. You'll have to check and see what New Jersey allows. As for your passport renewal, you won't be allowed to change the name on your passport until you can prove the name change is legal, either with a copy of the marriage certificate that contains your new name, or a court document with the name change.

When your marraige certificate is filled out you sign it with first name maiden name and your married name. That will be your legal name. Your middle name remains on your birth certificate but you dont have to use it.
