If your cat is underweight, you should get a vet check to amke sure nothing is wrong. Some cats are naturally smaller than other, of course. But if the weight is so low as to concern you- get a vet's opinion.
If there is nothing wrong medically- you cat is probably just naturally small.
If there is something medical causing the weight problem, it is possible that it may be remedied. However, some cat ailments are longer term and will cause ongoing low weight issues.
For example, "mega colon" prevents proper food digestion and may cause weight decrease. It is not uncommon in cats and it is usually a permanent condition. Also, other chronic ailments can have a perpetual effect on your cat's appetite- such as chronic renal failure.
Just because the condition is chronic and can result in weight loss, however, does not mean you cannot don something to help the weight stabilize.
I have an older cat (18 years) who was given 30 days to live nearly 2 years ago. She has mega colon (which dropped her weight from 7 pounds at her prime to about 6 pounds). Then she went into chronic renal failure. Secondary to this, she has had a bladder infection for 14 months that meds are not curing successfully. The renal failure and bladder issues added complications to her weight issue and she dropped to 4.5 pounds about 2 years ago.
She is not in pain, though. She is our alpha cat, and still lets the other two know and she remains quite active. And, we wanted to keep her around. She is on meds for everything and has stabilized, but anymore weight loss could be dangerous.
I put her on ensure plus (the adult high calorie dietary supplement), and I give her 1/4 bottle daily by means of a dropper (1/8 bottle twice per day). This has maintained her weight. Her appetitie is there most of the time, but because of her mega colon, I keep her on the ensure. Her weight stays between 4.5 and 5 pounds.
So, even if your cat had chronic illnesses- there are things you can do for the weight issue.
But first and foremost- have her checked by a vet!

Why do you want the cat to gain weight? if it is healthy and eating proper cat food it should not have a problem with weight. If it is a kitten or has been abused etc. you can add formula and goat's milk to the diet to add weight along with a good quality cat food. If the cat has just recently lost weight get it to a vet!!! NOW!! this can be a symptom of some very bad problems. (it can also be psychological or stress) but it's better to be safe rather then sorry.

Your cat may look skinny or too lean but may not be. If you can feel a somewhat thin layer of meat on his ribs then he's the perfect weight. If not, then try kitten food. It has more nutrients in it to help weight gain, that is why its given to pregnant cats. Or leave a bowl of food full so that your cat may eat when ever its hungry.
