So many people have been kind enough to answer my questions about Guatemala. I've been told the Vacination for Malaria can have worse side effects than the actual illness. Are the shots necessary or optional? Are all the diseases listed rampant there, only in rural areas?

Malaria is pretty bad. It will stay with you for life. Indeed some people react to the shots badly but that's only temporary.

A possible scenario:
You risk it and don't take any shots against malaria...but you get back and you turn out to have it. It will be a long way to get well again and your immune system will be seriously diminished. When you get a fever it will almost always be roaring high and you will be very ill even from a minor flu.

I'd take the shots.
