My husband said the note should read "Si es tan amable de dejar su número telefónico." Another option is "Si tiene la amabilidad de dejar su número telefónico."
I'm so incredibly picky about being grammatically correct and I don't like either option, but he said it's the best way to relate to the person to whom the note is being written. He admits it isn't grammatically correct, but it's better than being "formal". Are these options OK for the situation (just a short note for the housekeeper)? How would the "formal" and "grammatically correct" way be written?
