what is the legal implication of extracting blood from the fistula or gortex? who will be held responsible for that incident?

an AV graft should only be used for dialysis. it shouldn't be used to draw bloodwork for laboratory analysis, for instance. it's not that it's dangerous to do so, it's just that those grafts are so prone to clot off or malfunction that you shouldn't let anyone who isn't trained to access them do so.

as far as legal ramifications or holding someone responsible- there's nothing to do unless there were complications.

Actually i think noone should extract blood from a fistula or graft except a dialysis trained person. In an unusual circimstance a doctor might order for one to do so but i would think not. Did something go wrong?

In medicine, a fistula(e) are an abnormal connection(s) or passageway(s) between organs or vessels that normally do not connect.
Fistula may bleed. It is a pathological condition not caused by any human mistake. None is responsible
