Community service form?

I'm on probation and am wondering if I can just download the community service form, or do I have to drive all the way to my P.O. to pick one up. I plan on doing some hours next week and don't want to make the drive if I don't have to. just curious. I'm on probation in florida

contact your p o and ask them to send you one, also you can ask the department that you will be doing community service for, sometimes they have the forms already there

Community Service Form are accountable forms and may not be available on line. You have to get the form personally a the Probation Office.
