This is a traditional way of wishing someone a happy birthday; the correct expression is "many happy returns of the DAY", but it is more usual to say simply "Many happy returns".

I have not heard any of these three used in the southern United States where I live, and I have lived 37 years here. What I have heard used was simpler:

Many happy returns!
