What sorts interventions do you do for both?

You assess a patient as soon as you meet him. We do a head to toe assessment at least once per shift. Then you monitor them for changes in their condition or changes in your assessment of them.

My wife, the R.N., used to explain it this way.

When she assesses a patient, she examins him/her to determine what his/her current state is.

When she monitors a patient, she is collecting data to determine changes in the patient's condition over time.

Monitoring is "expermet and obseration" A monitor, and the eyes and ears of a medico are used. . Assesment is "inference" and the medico`s brain is used.
