Perforated colon is one of the risks of colonoscopy, but it is rare (approx 1 in 10,000 chance). It is more likely to happen with older folks, as they are just more delicate all over.
If a perforation happens, it's off to emergency surgery to have the hole repaired or that piece of bowel resected. The surgeon will also give the abdomen a good rinsing, to get rid of any bowel germs that may have escaped into the belly.
The good news is that patients are already cleaned out from the prep, and haven't eaten, so surgical risks are reduced a bit from other types of emergency surgery.
I've seen about 3 or 4 perfs. It's a bad sign when the scope is showing the OUTSIDE of the intestines instead of the inside...

If the perforation is severe or it changes the vital signs they will require immediate surgery.
Sometimes with very mild cases that are contained, we can watch and wait. Granted this is usually not a restful night for patient, nurse or MD, but sometimes we can spare the patient an operation.
Antibiotics are started prophylactically either way.

We are told to pray without ceasing. When we do that we are more in touch with those around us, because we pray for them. There is no reason that anything should happen to you. Many, many people pray on and off throughout the day. I'm one of them... When I walk into a classroom, when I'm helping students, when I visit the sick, when someone calls with a problem, they all become part of my prayers each day. There is no such thing as praying too much.

The scope instrument itself does not cause holes in the intestinal tract; it's a flexible piece of material and is gentle. It investigates what is going on in your colon. It is highly unlikely to cause any holes in your intestinal tract.
