Uhm most come from a decision not to wash your hands very often, which means that your hands touch things that are diseased, then get introduced into your body via your sticking your hands somewhere like your eye, nose, mouth etc. Then bam you are sick.
It's said that a cure for the common cold is to wash your hands frequently. Whenever there's a flu outbreak they also tell you to wash your hands. These diseases are not airborne, but rather are on surfaces.
Most diseases have a cause, some are caused by decisions, some not. For example if you make the decision to smoke, you'll probably get cancer.

NO, not at all somethings like you say we can watch out for, but we don't choose to catch a cold or cancer true there may be things we can do to help avoid (if we knew what is was). Skin cancer for example (avoid too much sun light). Chicken Pox, among many, many others

Sometimes it is a decision. Millions of people in the world aren't dying from AIDS because of some random event. That was a choice. Easy to catch a cold or flu without doing anything, but when we have knowledge to make better choices, then we have a responsibility to make good choices.

There are certainly some sicknesses that result from choices you make, but I think most are just the result of bacteria and viruses that you catch by happen-chance or that result of cell mutation that's not your fault at all.
