The tongue is the hardest thing to tame. The Bible says that if you can tame your tongue you can do anything. Sanctification is a life long process, and not one of us will be 100% sin free as long as there is breath in our lungs.
It takes 21 days to form a habit. I suggest you take the next 21 days and try a reward system. Have those around you help, every time you swear take something you really like away for 2 days, every day you do not swear reward yourself somehow, massage, new clothes, ice cream, whatever. Gradually stretch out the time you go without swearing. If you need to extend the time that is fine, you need to make a concentrated effort to stop.
But before you do anything you need to give it to God, I assume you have already prayed about your tongue, right??

This is what is called a besetting sin. You need to pray and pray and pray for God to help you. You can not have victory by yourself...you need Gods help. But you also have to do your part and do the very best that you can to stop. We do what we can and then God does the part we can't do. God knows your heart and knows if you really want to stop or not. Perhaps you can take it one moment at a time. Say, "For the next half hour I will not do it." And then, once you get through that half hour go for another half hour. Also, be careful what you allow to enter into your mind. Don't watch television shows, movies or listen to music that has bad words. If you are in an area where people commonly use foul language then wear a headset and listen to some good music and block them out. It has to go into your head before it can come out of your mouth.

I struggled with smoking for a long time after giving my life to the LORD. The thing that got me over it was when I thought I would not get to go to Israel with a Prayer tour because I smoked. I know GOD has called me to be a "watchman on the walls of Jerusalem" but I was about to blow my chance to actually go to Israel. You can imagine how important this is to me. When I made up my mind to quit, God met me there with a new pill called Chantex . It is a new drug that does not put nicotine into your system but works in the brain to block the need (in the mind) for a smoke. I had tried everything else but this is working. But only because I finally gave it to GOD.
You can do this too with your language. Just don't let the devil tell you that you can't make it to Heaven because you still slip with your tongue. Even the Bible tells us that the tongue is the hardest thing to control. You are already most of the way there.

Paul addressed this in Romans 7:19-20---"For the good that I want to do, I do not do; but the evil I do not want to do, that (is what) I practice. Now if I do what I try not to do, it is no longer I who do it, but sin that dwells within me."
v. 24-25:"O wretched man that I am! Who will deliver me from this body of death? I thank God-through Jesus Christ our Lord. So then, with the mind I myself serve the law of God, but with the flesh the law of sin."
Though a believer is delivered from the guilt and penalty of sin, they will not be free from the presence of sin until Christ returns. The struggle against sin will continue throughout life on earth. The difference for a Christian, who has the mind of Christ, is that there is the hope of being rescued from their sins, and the power to achieve victory over the sinful nature. Though the struggle with sin remains, we are no longer condemned before God, but stand righteous before Him.
We will always struggle with sin. For me the best way to overcome a habitual sin is to pray for God's help, ask His forgiveness when I slip up and not to make excuses for myself. God will help us to overcome habitual sin. -Remember, He who is in us, is greater than he who is in the world. We can also "practice" using self-control: as they say. practice makes perfect. lol :)

† If memory serves me right isn't it Paul that struggled with sin... Asking why do I do that....
I think you struggle with it because we all have some cross to bear... Sin, sickness, struggles, depression.
But be of good cheer, He has overcome the world...We are not alone nor are we the first. All have fallen short of the Glory of God... All have Sinned.
Pick yourself up every day and start afresh. Do you best to do what you know is right and be good to others. Put the past behind you and look forward towards the finish line... Pay attention to where you are going... GBU

Pray and ask God to forgive you. Then remember that you are an ambassador for Christ. Do not grieve the Holy Spirit.
Usually I use a scripture that comes to mind or I start singing a praise song after I rebuke Satan in the name of Jesus. And remember you can do all things through Christ who strengthens you. That is the scripture I use alot.

James said the way to control any part of the body is to use the tongue.. put pressure on the body by use of Gods authorative word.
this isnt so strange as it might seem.
in all areas of secular life, people speak before they do something. and what they speak becomes what they do. so the area of saying bad words is essential to change. (not that i'm perfect everyone is making changes by degrees)
look through the Bible for every scripture that addresses the matter of speaking rightly. thinking rightly. speaking God's word for good effect. loving others.
then take your word medicine each day and keep that word in your thinking, in your mouth throughout the day.
start with one verse per day. then increase the scripture dosage when your ready.
decline to engage in communication which goes against the scriptures that you focus on.
and refrain from correcting others when you are in your program of heeding God's correction.
what your doing is changing your direction. not changing the tools you've been using all this time.
when that word abides in you. it will come alive and the Lord will make his presence and approval known to you.

It's wonderful that you seem very motivated. Habits can be difficult to break, but it's possible with God's help. We must do what we can and God will help with the rest. You most likely know the steps for repentance, but I went ahead and listed them along with some extra recommendations in case you'll find them useful.
Steps of repentance:
- Admit to God that we have done wrong.
- Feel sincere sorrow for our sins.
- Ask God to forgive us.
- Do all we can to correct the problems our actions may have caused.
- Turn away from our sins and never repeat them.
Other notes:
- Don't watch any TV or movies with bad language
- Avoid people who use bad language - if you encounter it, ask them to please not use bad language in front of you
- Pray each morning for the strength to keep your commitment
- Report how you did through prayer each night
- Forgive others - If we don't forgive others, God won't forgive us
- if you ever slip up, appologize to whoever hears you and start over with the above steps
- Sometimes people fail to commit to turn away from the sin because they don't have faith they can keep the commitment. Regardless, recommit and have faith that God will help you with what you can't do by yourself.
- Don't let yourself be discouraged when you fail -- just recommit. As you do these things you'll develop a new sensitivity to bad language, which will help you avoid it.

I used to be 'just like you' and I finally 'kicked the habit' of putting swearwords in my sentences. It mostly takes a lot of listening and thinking before I said things. I practiced saying things that would glorify God instead of things that would dishonor Him.
Habitual sin is something that yes, you can overcome.... but like i said, you have to conciously choose to NOT do it.
God knows your heart... and if you truly are sincere about this, pray and ask Him for help... ask God to lift off this burden from you and He can and will do this if it is what He wants from you. But you have to be striving for it as well.
Take one day at a time.... always pause before talking/speaking and continually ask God to help you speak only Glorifying words from your lips
