Probably not when placing an ad on the internet, but probably within the first couple of dates. Think about it, if you were deaf in one ear, would you want to post that and have potential persons to date decline for only that reason?

You should disclose items upfront that would cause someone to feel that they were tricked if you didn't disclose.
In the case upfront does not absolutely require that you tell them in the personal ad. You could tell them in the first or second phone call, once you've screened them a little bit.
Some things are so big they would require an immediate disclosure. For instance if you were born as a man and had a sex-change operation then you have an obligation to disclose that in the ad.
If you are on house arrest and have to wear an ankle bracelet then you should disclose that within the first two phone calls.
On the other hand if you are missing a breast then you could disclose that after a dozen phone conversations and it should still be OK.
In general let the golden rule be your guide here.

I would maybe MAYBE warn if you are blind or deaf but sharing that you only have one breast is inappropriate to the extreme! Too personal even for a fifth date!!! Unless you are slutty and plan to have slept together by then. I also wouldn't advertise I am too fat or I am only 5 feet tall. If you want to get than out there use the right wording. You know what I mean...words like voluptuous were made for people placing these kind of adds.

I think someone should include the information if it's something major. If you post an ad with the intention of meeting someone, they're going to find out eventually so you might as well be open about it from the beginning, within reason.

Absolutely you want somebody to love you for whats inside not what you look like. And if you wait it might come as a huge shock to the person who might be falling for you and scare them away. Beauty is really an inside thing, that person should show how perfectly beautiful they are dispite minor blemishes. Everybody has them and finding somebody to look past them is probably the best thing to ever happen.

I've seen people who DID list their downfalls.
It just shows me that they are honest and open about themselves and life in general.
It never turned me off.
You would probably attract other honest people as well.
However, you WOULD miss out on a lot of potential mates by listing them. It might scare away someone who would actually be able to look PAST your downfalls once they got to know you first.
It's a double edged sword.

perhaps not up front --- you just cant leave it too long --- people are too preoccupied with appearances so let them get to know you a little before you tell them
