There is a hole in the bottom of the doughnut, you just don't see it because there's usually a glaze that covers it. When putting in custard or jelly in a doughnut, they usually get a pastry bag with a tip and insert the tip into the doughnet. Then a specific amount of filling is injected into it and there you have it: a luscious jelly or custard doughnet!

They have a machine and the stick one end of the doughnut into the machine and push the button. The jelly goes into the doughtnut and viola...a jelly doughnut. It's really easier than some people think it is. They make custard doughnuts the same way.

i work for tim hortons and we use a machine that injects the filling sometimes the whole is hard to find but its there along the side

When the donut bakes up, it has a hollow center. So, they just cut a small hole in the bottom, and pipe in the filling. Glaze it up to cover the hole, and voila.

Sometimes the hole is hard to find, but usually it is there if you look hard enough. Either that or it's put in there before it's baked.
