My previous employer hasn't provided me my W2. I had worked with them for 9 months in 2006. They also never provided me with a pay stub, so I have no idea of what my withholdings are. What are my options of obtaining a W2 with all my withholding information. I contacted the IRS and filed a complaint about this and they told me that they would send a letter to my previous employer. But this hasn't produced any results either.

Contact the corporate office of the employer......they will direct you to the appropiate office. This happened to me back in 2004...but after speaking with human resources and payroll..I got one over-nighted to me.

Ask your prior employer. They are required to do that. Make sure you were paid as an employee and supposed to get a W-2. You may have been paid as an Independent Contractor, where there was no withholding, but you should have received a 1099 (instead of a w-2).
Ask your employer about this. Go to the company. Don't leave without it.
After all they are not gonna pay your late fee with the IRS. You will.

All you can do is bug the employer. If you don't receive it in time to file on April 15, Enclose a letter with your return explaining it.
Since you do not have any stubs, report to the IRS the net earnings, take home amount. Give them the exemptions that you claimed. They will be able to back into the gross and the witholding tax, SSI, medicare, etc.
The employer will be deep s*** if they have not remitted those to the gov. They will have to pay all taxes etc. plus fines and penalties.
