A Chinese doctor that's also a karate master? Are you sure you have that right? Karate and Chinese medicine doesn't normally go together.
A banner or plaque of appreciation, a calligraphy piece, etc., would do nicely. No edged weapons or time pieces.
*Edit: Being 'sloppy' just shows that you don't really care or respect the person and his/her work. Why even bother pretending that you care with a gift?

If you train with him you may look into a fine broad sword. This is a traditional weapon in Kung Fu and you can pick up a very nice practice sword for minimal cost. You can spend a lot of money if you wish, but a fine example of weapon can be hand for a few hundred or less in most cases. I recommend speaking with Rafael at Valley Martial Art Supply. He is a sword expert and is an invaluable resource to martial artists.

the best way to show him respect would be to brew him a cup of chinese tea.....and offer it to him with both hands outstretched.....it is a great mark of respect to a sifu.....
