When two friends become too close to an informal level than it is normally used for each other and they both never mind.
But its meaning is not good. The two good friends, when they use it to say each other, they don't really mean it.
It is just used as an idiom.

It's an insult/abuse but many pakistanis guys use it in their informal conversations. It means "sister-*****r"
