Korean Airlines flight 007. The History Channel program suggests that a possible cause of this incident had to do with the Korean social structure. Allegedly a subordinate crew member knew that the plane was going off course, but would not tell the pilot because it would have embarrassed him and caused him to "loose face" with the other crew members.
