Maybe you should start with the bible and the prayer that Jesus taught us the Lords Prayer/ Our Father, and yeah prayer from the heart is the most pleasing to God.

Or alternatively you could go along to your parish church and see if there are little prayer booklets for sale most will have them.

Well you may start with an act of contrition or asking God pardon for any sins you committed during the day. An examination of conscience would also be nice to do. Or you may just thank God for all the things He did for you today and everyday. You may thank Him for your life, your family, friends, food, house, etc. Then you can tell Him your secrets and ask Him for things you need. Prayer has four main dimensions: contrition, thanksgiving, adoration and petition. You may do any of these any time and all the time, not only before you go to bed. You may begin to see God as your Father, Brother, Best Friend, Everything! He knows you in and out so call to Him anytime and you will see that He will help you out many many times. Then you may also ask Him for a good friend - a catholic friend or a spiritual director who can help you establish some form of friendship with God. You may also choose a good spiritual book or the bible. Ask a relative or friend to help you out. Ask the nuns or priests, or anyone who knows how to start a relationship with God. It may have its difficulties but it is the best adventure and decision of your life. Ask Mary also to help you and your guardian angel too. Read also the lives of the saints. They are awesome. Their life will show you how to love God. Then you may also learn to say the Morning Offering everyday, which is offering everything you have done to God in union with the Mass. Offer God all your past, present and future words, thoughts and actions. Then your whole life will be useful for God's glory. God bless you.

Going to Church would provide you with many examples for praying.
Perhaps, at night, you could recite on decade of the Rosary.
Have a favorite (verse in a) Psalm? Recite it to yourself or outloud when you think of it during the day.
Allocate a portion of your day for Bible reading. This is a form of prayer as well.
There are countless ways to offer prayer to God. Really, the only limit is our imagination.
The hardest part, in my opinion, is keeping up with regular prayer. They say it takes three weeks to form a habit. If you are disciplined enough to pray (a decade of the Rosary once a day for example) for three straight weeks, you've overcome the hardest part of regular prayer.

You could begin with this prayer in the morning:
Eternal Father, through the most pure hands of Mary , I offer thee my prayers, works, and sufferings, all the actions of this day and of all my life in union with the masses said all over the world for the Intentions of thy Sacred Heart and the Apostleship of prayer.
Every breath I draw, every beating of my heart, every glance of my eyes, every step I take , every single act I do I wish to be an act of love for Thee."
At night before bed you could say an Act of Contrition:
O my God, at the end of this day I thank you most heartily for all the graces I have received from you. I am sorry that I have not made a better use of them. I am sorry for all the sins I have committed against you. Forgive me, O my God, and graciously protect me this night. Blessed Virgin Mary, my dear heavenly mother, take me under your protection. St. Joseph, my dear guardian angel, and all you saints of God, pray for me. Sweet Jesus, have pity on all poor sinners, and save them from hell. Have mercy on the suffering souls in purgatory.
Begin praying in a small way and slowly you will receive grace to pray more . As you feel closer to God start praying the Holy Rosary. If you find it too difficult to say five decades everyday, just start with one decade everyday and move on to saying the daily five decades Rosary (or all fifteen decades!) everyday. Soon you will receive grace and begin going to church. Go for a confession before you receive Holy Communion.
God is waiting for you with open arms!

Prayer is the elevation of the mind and heart to God:
+ In praise of his glory
+ In petition for some desired good
+ In thanksgiving for a good received
+ In intercession for others before God
Through prayer the Christian experiences a communion with God through Christ in the Church.
"For me, prayer is a surge of the heart; it is a simple look turned toward heaven, it is a cry of recognition and of love, embracing both trial and joy." (St. Thérèse of Lisieux)
"Prayer is the raising of one's mind and heart to God or the requesting of good things from God." (St. John Damascene)
But when we pray, do we speak from the height of our pride and will, or "out of the depths" of a humble and contrite heart? (Psalm 130:1)

You can always say an "Our Father" if you are out of words, but its best if you speak to him from your heart.
Other prayers you could say:

Try going to Church. You'd be surprised how life changing it can be :)
God bless!

Just start talking to God.
Talk to him all the time.
Now, if you want MORE ... you might go to your local parish priest and ask. He might advise you to join thier RCIA class. Get you back into the Church. He may also advise you to get a "Spiritual Advisor", and he SHOULD have suggestions for you.
God Bless you in your efforts.
