You should watch the movie The Da Vinchi Code. Maybe there you can get answer though I haven't watched the movie yet but I have heard a lot about it.

What it means/What Lisa is smiling about. Though, its beginning to be understood as resolved. A few years ago some scientists ran a scan over the picture for things visible not to the naked eye, and they found a transparent gown that was very commonly worn at the time for women who were pregnant. Thus, perhaps her smile is that she is going to have a child.

Not conventionally pretty, she seems to almost be smiling, yet we wonder what she is thinking. She does not have the blank or focused off look of most portraits, but looks at us. And Leonardo dragged it around for years.
