The country where the account exists as well as any other country that taxed the income.

If your account is at a US branch, the interest isn't taxed elsewhere and you don't need to use Form 1116 for that interest.

If the 1099 does not show specifically what country the dividends originated from, show "Various" as the Foreign Source.
