I receive dividends for 39 dollars but not got the form? Should I report this income? how?

All you have to do is report the income. There's no penalty or flag on your account if you don't send in the 1099 with it.

Sometimes the documents sent from the investment company do not look like 1099DIV but in fact are. However, no 1099DIV is necessary for you to put the dividends on your return.

Enter the dividends on Line 9a and 9b. If the payment is from a domestic stock or mutual fund, it will be qualified dividends.

If you have over $1,500 of dividend income, use Schedule B.

You should report it anyway, on the dividend line of your tax return. But I'd call the company and ask why you didn't get one. If the stock is held by a broker, call the broker.

If you got something called a "consolidated statement" from a broker, read through that, the dividends might be there.

IF THE AMOUNT IS LESS THEN $10 THE COMPANY WILL NOT SEND ONE. IF YOU HAVE GOTTEN ONE IN THE PAST, AND IT IS USUALLY OVER $10, I WOULD CALL THE COMPANY AND TELL THEM YOU DID NOT RECEIVE IT. IT COULD HAVE GOTTEN LOST IN THE MAIL.
