Hmmmm... This is a tough one. I will give it shot though.
Quit masturbating. Just a thought. It is suppose to be pleasurable not painful.
Seriously, sounds like the chemicals released during that activity is throwing your body chemistry off. It will take blood work to determine what is going on. Embarrassing but it has to be done. You could fib a little and say it is after sex with your partner, lol.

Fear in mind can cause this problem.
Creation of vaccum can cause this problem.
Stagnation / obstruction in blood flow can result it.
