Some people are naturally thin, and can eat a lot and stay thin.
I was like that all my life until very recently. I'm 40 now.
When I was a teenager, in my twenties, I ate a great deal of food, and still didn't put on weight. I'm the same height, 5' 7",
and I weighed 98 pounds. I was very active, and healthy.

I doubt it's a parasite. And no, it wouldn't show up on an x-ray.

Yes, it's possible. Tapeworms can do this to people, but tapeworms leave segments in feces, which are easily detected by people who are trained to see them. Chronic diarrhea can cause a lot of weight loss, and that can be caused by parasites such as tropical ameobas. Having an overactive thyroid can also cause severe weight loss.
In general, parasites can't be seen by x-rays. They are detected by stool and blood samples and by tissue biopsies.

You can't tell from an x-ray. I believe she would need a stool analysis or blood test.
