...a template. I'm noticing various worksheets that he has given to me with MY copies of the returns over the years. Is this something that I have to provide to IRS with my 2006 return, or did he just provide those for me and NOT file them with my returns?

Worksheets don't usually get filed with the return. Forms do. Each page should say at the top which it is. The applicable line on the 1040 will usually say "attach schedule __" or "attach form ____" - if it says to attach something, then you send that form or schedule with your return.
If you're efiling using tax software, the software will know which to transmit.

If there is an IRS form number shown on the page, it gets filed as a part of your tax return.
