Everything would depend on the extent of the damage of the puncture. If it is very mild and not associated with any other injuries it could just be unnoticed and just heal over time depending on which organ is affected. If severe puncture and causes internal bleeding, condition would deteriorate shortly if there is hemorrhage leading to shock. There are different things causing to puncture an organ. It could be secondary to other injuries, that is getting a penetrating injury either from an external object or a fractured bone for instance. Ingestion of sharp objects just like pins or bones is another cause of getting an organ punctured. No definite outcome with a definite period being known on its consequences. Situation and condition varies. You may tell there is bleeding or not and could die instantly , on the other hand it could also take sometime to cause problems such as having perforation which could be found out later. Is there is suspicion of having an organ injury, proper investigation should be done, that is the best thing to do.

Depends which organ. Brain bleeding is probably the worst.
Pain, pressure, drastic changes in blood pressure.
Some organs can heal themselves, some need surgery.
In doubt - Go to the ER they can help.
For Example:
A 6 year old little boy came into the ER after falling from a tree. He had a moderately severe liver laceration. He was kept still in a hospital bed and recovered in 3 days without surgery.

a "hemmorage" can kill you in a minute or over hours or days...depends on the severity.
sometimes there are signs on the skin or pain, normally a hospital now will do a quick cat scan, takes a few minutes and they know for sure.

You would be able to tell in a day or so because you'd start coughing up blood.
