Brother, a similar trick was used by sailors from the USS Pueblo when they were captured in the sixties.

When they wrote their confessions, they added the names of cartoon characters, dead celebrities and sports stars as their accomplices. That way, they let the people back home know that it was forced.

Even without the ability to speak freely, a hostage sending out a written confession has ways to communicate with his or her people. Right now, either the letter is written by an Iranian official (doubtful: they would have people who are extremely proficient with the English language do that) or it could be a sign it was written under duress.
