The same way most of us do they get paid for their work.

My sister is a certified goldsmith and a jewelry designer. She is an independent designer and seller who only makes money when she sells a piece. If she worked for a company that produced gold work then she would be paid for her time. Often on assembly line work the person is paid for the piece. If she were actually designing work then she wouldn't be paid this way, but if she just applied gold leaf then she might be paid by the piece.

They make money the same way any merchant makes money.
Buy low....sell high!
It's really all about the margin in the middle between buying and selling that dictates the high or low profitability of anything.

They buy gold from miners at 80% of gold spot price.
They also buy scrap gold(cheap gold watches, bracelets) at below gold spot price. (sorry i dont know what percentage for scrap)

They then make some new jewelry out of the gold and sell it for double or triple what the gold would be worth if it were melted down.
