The amount of your 403 (b) contribution is not included in Box 1 of your W2. The contributions were deducted before taxes were applied to your remaining wages, so you have already gotten a tax relief on those contributions.
This would not be reported on your 1040 and a 403(b) is a pension plan not an IRA

No.

They come out of your paycheck "pre-tax". You have already received the tax benefit from the contribution.
