Trust me, if he REALLY wanted to call you he would not have "lost" the number.
Never make the first move it WILL imply you're desperate.
More than likely he met someone better after you, he went back to an ex, he didn't like you as much as he made out, or he simply hasn't thought about it.
Try the number annonomously, bet it isn't really his.

Call him definately. This happened to me before and the guy in question had lost my number so I'm glad I went for it.

There are two main possibilities here. Either he isn't interested or he simply lost your number - or his mobile with your number in it. Give him a call if you are really keen and gently rib him about his memory.

Of course he hasn't lost it. He hasn't called because he might not be interested at this time. Don't call him. If he doesn't call you, it's his lost. You're better off without him!
