Need to know what "Kitchen Hygiene Certificate" looks like and different samples an designs of the same also how does one certify a hotel kitchen for hygiene

I can't help you with the samples, since there are so many different kinds, depending on the state. To get certified, you have to call the local branch of the health dept. or inspections and appeals. An inspector shows up, goes through the place, and lets you know what needs to be changed or fixed. If you pass, they give you the certificate on the spot.
