is it like the first step for all wanting-to-go-to-med-school people? or, what is it??

In the USA you must get a BS or BA degree before you apply to medical college. You can major in anything you want as long as you take the pre-requisite courses that med schools demand. Most applicants probably major in biology or chemistry.

There is no official major "pre-med" but in many schools they have some kind of a program to support kids who are getting ready to do the difficult application process for med school. So the college will have a pre-med advisor who knows how that works and who will give advice to the students who want to apply to med school. Students register with the premed advisor to be sure that they get the latest information about how the med school application process is working.
