Handfastings and weddings can have similar gifts, they both symbolize the coming together and uniting of two lives in love. Something that your friend might appreciate might be a seedling for a tree that could be planted in their yard to symbolize their new life together that will grow as one.
A large rose quartz crystal is always nice as it is a stone that metaphysically is for love, compassion and understanding. Or two crystals that are the same that both can carry with them.
Candles, incense are always nice gifts.
Otherwise, maybe the couple has a registry for traditional wedding gifts.

Go to the link below. I think it's a perfect gift for a handfasting. Due it being a candle obviously, but also the representation of the sky-clad couple, and it's a beautiful piece.

You could get them any gift that is appropriate for a wedding. The handfasting ceremony is a pagan ceremony, but is very much a wedding.. find out if they've registered somewhere and take it as you would any other wedding.

When are they getting handfasted? It wouldn't surprise me if it were near one of the Sabbats - if they follow a Tradition that celebrates them, then a symbolic or appropriate gift that reflects the holiday might be a good idea.
The thing is, "pagan" is such a broad term that without knowing a little bit better what branch of paganism, it's harder to name a meaningful gift. What a Wiccan would consider "meaningful" might differ from an Asatruar or a Druid or a Shaman, for example...

Anything you would give anyone of any other religion for a wedding gift is fine. Pagans do tend to love candles, incense, essential oils and thing like that, you could put together a gift basket of such items.

you should find a few ideas on the site above,
hand fasting is a very ancient pagan ceromony, that in times gone by, meant that couples were betrothed for a year, these days its usually for eternity, its often the "rose ceromny, where couples echange roses, & rings, & recite vows, pretty much as in a modern wedding ceromony, a lot of the traditions surrounding a modern wedding, come from the old pagan ceromony.......

Yes, wouldn't a skyclad wedding be magnificent! Most pagans bottle out of that. I think it depends more on the branch of paganism - it could be anything from a simple white cotton dress to a fully Halloween horror outfit. You know your friends better than I do - maybe you could ask them?

Being an Asatruar (it's a type of pagan), myself, I know a lot of pagans.
Anything you'd give at a wedding would be appropriate. See if they're registered.
If they're not, then I can tell you that nearly every pagan I know would be very pleased to receive something you made, yourself. It would likely have special meaning for them for the rest of their lives.
