yes there is. severe bruising of your genital area, with possible blood blisters from your penis smacking your nut sack around.
if anything wear a leopard man thong. yeah... skip the tux all together..

Aside from a possible zipper accident, then no it is perfectly safe. Though being as it is a rented tux and you cannot be 100% sure that it was properly cleaned prior to you wearing it, I'd suggest a pair of loose boxers. You don't want to have to think that your tool is brushing up against material that some other guys junk might have been resting the night prior.
But medically speaking, if the clothes are clean, going commando is healthier than wearing underwear.

I do not think their are any health implications that wearing a tux while commando will cause. However, if you already have health issues, like leakage or anything like that I wouldn't advise it. I mean its rented. Some stains you just can't get out. Hmm, now that brings up another point. What about the person who wore it before you? How would you feel if they went commando and had health implications.
