Dandruff is created by a condition called seborrheic dermatitis that produces an overabundance of oil (sebum) in the skin, but also creates the flakes, from irritation sloughing. The skin can often be greasy and scaly in addition to flaky. But even if it's just flaky, don't assume that's because it's dry! Trying to moisturize can make it worse, even though doctors sometimes prescribe a steroid cream and/or an antifungal cream to clear up the same condition on your body or face---the key is those are steroids and antifungals in addition to being creams.
Since it can be caused by fungus as well as by hormones or no particular reason, doctors usually recommend an antifungal dandruff shampoo like Nizoral (which now has an over-the-counter strength in addition to a prescription strength). If you do buy that one, get the larger bottle (open the box of the smaller one to see how small it is and you'll know why!) Please don't try to condition it away! Remember, moisturizing can make it worse. So can overwashing with the wrong shampoos.
Nizoral is the absolute best, and the one doctors recommend most, but if for some reason you can't find it (I've seen it at CVS, Walgreens, Duane Reade, and Safeway, so it shouldn't be a problem) or don't want it, try a dandruff shampoo with an ingredient like pyrithione zinc (Pantene Anti-Dandruff and Head & Shoulders). Neutrogena's T-Gel uses a tar chemical instead, and that works for some people. Selsun Blue is another that people like. There are also ones with salicylic acid in them, but they just exfoliate, and for a case as severe as yours sounds, I wouldn't recommend it. I strongly urge you to use the Nizoral, actually.
You can read about it here:
http://familydoctor.org/157.xml...
but it seems that the doctor wrote the recommendations for treatments before Nizoral was made non-prescription. He talks about it in the section about seborrheic dermatitis in babies, but please trust me, it is what doctors recommend for severe dandruff in adults. And you can see it's gentle enough for babies with that recommendation. :)
I've used several different types of dandruff shampoos over the years, and though some have worked well (I do like Pantene Anti-Dandruff), Nizoral is the best, fastest, most amazingly effective treatment I have tried.

Dandruff is one of the major disorders of the scalp.Poor hygiene, improper diet ,sluggish condition of the scalp caused by poor blood circulation are the main causes of dandruff. A herbal rinse is a safe method of removing dandruff. Apply lemon juice with the rind on the scalp .After 5 minutes wash off with shampoo and water. Tea Tree and Eucalyptus oil shampoo is by far the most effective and environmentally safe method. The active ingredient in (Clinical Strength) Head & Shoulders is "selenium sulfide". With the exception of Head and Shoulders Clinical Strength Shampoo, the rest of the line of shampoo products specifically contain "pyrithione zinc" as the active ingredient. While neither active ingredient is highly toxic, humanity often fails to consider the combined post use soap in the sewers and both these compounds break down slowly, where the natural oils are harmless. Hope that helps.

It depends on what's causing it. Dandruff is flaking skin but flaking skin can be cause by psoriasis, seborrhea, dry skin and more. You need to look at your scalp and see what it looks like to determine what the cause is then you'll have a better idea how to manage it. Check out this link: http://www.coolnurse.com/dandruff.htm
The best way to manage it is to rinse your scalp every day and scrub it well but skip the shampoo every other day.
