For the past thirty plus years, second generation feminists have taught that there are absolutely no differences between men and women and that any traditions, such as chivalry, which recognize the inherent differences between men and women are "sexist" and therefore "wrong".
Thus, second generation feminism is absolutely responsible for the death of chivalry.

Chivalry has been killed because it's just stupid. I had a guy actually take his shirt off on a date because it was raining and he was going to put it over my head. Give me a break!

No, you can't blame it on feminism, blame it on the men themselves who seem to have forgotten how to treat a lady. It seems like anything that requires time and effort (and doesn't involve sex) men shy away from. Its ridiculous besides I think you have the definition of a feminist misconstrued.

Absolutely not!
I'm still quite a traditional girl. I love it when my boyfriend opens doors for me and such. We usually take turns paying, but if he offers to pay I'll gladly allow him, most of the time. =P
It's a lot of the small gestures that say a lot of about a person, and when my boyfriend is chivalrous like that all the time, in public or in private, it really tells me that he cares about my feelings and wants me to feel appreciated.
