Since creatine is metabolized by the liver, and since Accutane specifically has been linked to possible liver damage and hepatotoxicity, I wouldn't. A lot of people have their liver functions tested all the while they take Accutane to make sure nothing happens from it. I wouldn't casually throw something into the mix, even at a small dose, without at least asking my doctor about my case in particular. And certainly not something that specifically acts upon the liver when Accutane is known to cause complications there under certain circumstances.

I want to point out that my tone here is that of a suggestion, because I'm not a doctor. It seems like common sense and caution to me. But for all I know there could be a dire risk in doing this, and possibly your personal medical history would make it even more dangerous. You don't want to end up with a liver condition that will endanger your life because you did this. Please don't do it, or at least ask your doctor first, so s/he can tell you and/or be aware to monitor your liver function more closely.
