Actually, at least in Catholicism, grave sin and mortal sin are the same. I think you're talking about Venial sins. If so, the difference is that a mortal sin:

1 - Must be of a subject of serious matter;
2 - Must be committed with full knowledge of both the sin and the gravity of the offense; and,
3 - Must be completed with deliberate or complete consent.

In Catholicism, dying with a mortal sin is a guaranteed way to land in hell.

Mortal sin is the sins of the 10 Commandments. they are able to be forgiven by potential of confessing them. Venial sins are smaller sins that still may be confessed (yet do no longer could be) and don't bar you from the dominion of Heaven could you die with them on your soul. yet another huge distinction is that if a Mortal Sin is on your soul, you are able to't take communion, while with a venial sin, you are able to.
