Something related to something she likes, but with some extra significance. If a movie or album she really likes is coming out, and there's a special edition with extra footage/tracks/art or something like that, get her that one. If there's a concert, perfomance, or sports game she likes and you can swing tickets, that's good too. If you don't have a lot of money, get something small that she'll like or is connected to something you share as a couple (something you did, some joke you share), and make her a card. Anything that shows thought. Try to steer clear of jewelry unless you're sure of what she likes - some girls can be very picky.
I don't know what sort of relationship you have or how long you've been dating - I'm assuming not that long if you don't know what to get her for her birthday. If she's the romantic type, my advice still stands, but dress it up a little - bring her flowers (if she likes them and isn't allergic), take her out to dinner at a nice place (that she likes, and don't go overboard), something like that.

Here's something creative that will be appreciated and you will have fun doing:
Make up a crossword puzzle and have all the hints be personal inside moments and jokes that only you two would know about. Put it on a poster board and add some clipart and some pictures to it!
The crossword puzzle could be time consuming and a bit frustrating ... you could do the same idea with a WORDSEARCH or WORDFIND puzzle.
Put your computer skills to work! Use Excel, get clipart from Yahoo Images Search, copy them, and reformat them, use a gluestick to put on posterboard.
Use graph paper, and pictures from magazines if you are not as good on the computer, and it will give it a homemade feeling.

Make her a blog/timeline telling her how you feel about her and talk about all the great times you had together. I used http://ourstory.excellentabc.com to make one for my g/f and she loved it. Best part is it is free to do and you both can keep it updated as your relationship grows.
