i put the note in his locker with my s/n but what if it feel out when he opened it and he didnt see it?

Then he probably doesn't know your screenname? Why don't you just walk up to him when your sick and cough on him, it will be more effective, I mean I don't mean to a professor, but yeah I do teach some classes at a community college, and this thing happens all the time. If you don't know what to say, just wink but try not to do it with both eyes at the same time cause then you will just be blinking at him and no one wants that, I know i don't because one time at the college there was this girl that blinkied to much and both of her eyes fell out, and it was so strange.
