Or, is there a way to have the state of Kentucky notify the senior that they need to test them at random and give them a new driving test? This is done in some states but I can't find anything on Kentucky. The notification is usually done by a doctor's office or relative.

I believe you are correct that the request must come from the driver's doctor, or a family member having knowledge about the driver's driving habits. Families very often can have the elderly person's license taken away, if they feel the driver or the public is in danger.

Contact the KY DMV and ask them. State that you are concerned for the safety of this individual and others on the highways.
