It's not that tough. Put a wooden frame for the sides, with the top being level with the top of the walkway, and layer about 3 " of gravel underneath. If you want extra strength, you can put some steel mesh spaced so it'll be at the center of the concrete pour. Have some large trowels ready to smoothen the top.
If you want a nice effect, add some colored gravel on top, and use a setting retardant on top of the concrete, so you can hose off the top and have a nice sculpted effect.
