Many things can contribute to poor posture; weight (i.e. large breasts), lack of muscle in the abs and back, and poor self-esteem, believe it or not.
Your back was not designed to be PERFECTLY straight. There is a natural curve from front to back, not side to side, which is a sign of scoliosis. However, if your shoulders are slumped forward or your butt sticks out too far from an arch, these are signs that your posture is bad.
One thing to do for better posture is to strengthen your back. Lie on your stomach, strech your arms on the floor over your head. Now lift your arms and feet at the same time so you look like the bow or a little U-shaped. Repeat this for back strength. Strong ab muscles are also important for a straighter back so start doing crunches if you're not already.
A good stance involves tucking the hip bones under the body. So it actually feels like your rolling the bottom portion of your pelvis forward. And keep those shoulders back and down. Don't let them roll forward and keep your head held high.

It could be weight related. I would say it's either weight(body weight or booby weight), curvature of the spine, or from not sitting up straight for an extended period of time(slouching). If it's curvature only a doctor can correct it. Otherwise, just make a point of sitting up straight. Walk around your house with a book on your head.

I'd try a back brace. Weight could be a factor. Are your boobs big...they could be causing you to hunch over putting pressure on your back. If that's the case, then maybe you should consider a breast reduction. You should speak to a chiropractor to determine the cause of the problem.

Practice sitting straight. It will be uncomfortable for a while but will eventually become habit. It could be weight related if you have overly large breasts.

Strengthening exercises for your core. Yoga is also great for the back. Practice standing straight as well. It won't happen overnight, but with just a little work, you can have great posture.
