Today is my man's birthday?

What should I get him? He's already recieved his actual presents- Opening day Red Sox/Royals tickets and a PS3.
I want to give him something small today..
I was thinking a birthday cake and balloons- not sure if that's really a "guy" thing?
Any (serious) suggestions would be greatly appreciated!
